Care Coordinators
Care Coordinators connect patients with the help, care and support they need to manage their long-term conditions and to make decisions about their own care.
This service can help patients who are over 18 years if they have one of the following:
- have one or more long-term conditions, such as diabetes, asthma, dementia, chronic pain, or frailty
- are an unpaid carer
- have a cancer diagnosis
A care coordinator can help patients navigate the health system and access local services. They will help you with a care and support plan that is centred around what matters to you. They will work closely with you, your carers, and the important people in your life to make sure you receive the best possible care. They can offer support to newly diagnosed cancer patients and help people to access routine cancer screening.
