Mental Health Social Prescribers
Our Mental Health Social Prescribers act as a key point of contact for patients with mental health symptoms, such as low mood or depression, by giving extra support and providing guidance and advice to help with self-management. They can direct you to non-medical support in the local area, such as wellbeing groups, physical activity groups, and self-help. If needed a support plan will be developed which will include workshops or one-to-one meetings. If needed an onward referral can be done for patients who need extra help with their mental health.
Mental Health Pathfinder
Mental Health Pathfinders help adults with mental health symptoms whose needs cannot be met by local talking therapy services, but who might not need ongoing care from hospitals or mental health teams. They improve mental health support by providing guidance and advice about self-management or access to treatment services if needed for symptoms such as low mood, anxiety, and depression. They may consult and conduct psychological interventions where qualified to do so and where appropriate.
Youth Emotional Support Service (YES)
The YES team offers support to young people aged 11 -17 years. They support and empower young people who are experiencing mild to moderate mental and / or emotional wellbeing difficulties to make changes to the way they are feeling by offering them strategies, advice and guidance.
